The Licence Appeal Tribunal found that the applicant failed to comply with seven requests for information without a reasonable explanation.
Consequently, the insurer is not liable to pay the claimed benefits.
The Tribunal also ordered the applicant to pay $250 in costs for pursuing an unreasonable appeal.
