The Registrar issued a Notice of Proposed Order to refuse the registration of the corporate registrant as a gaming supplier.

At the hearing, the parties presented an Agreement and Joint Submission Regarding Disposition, in which the registrant admitted to not acting in accordance with the law or with honesty and integrity.

The Board accepted the joint submission, ordering a 60-day suspension of the registration and imposing conditions requiring compliance and reporting of any charges or administrative actions.

The Board previously found that the licensee violated subsection 45(1) of Ontario Regulation 719/90 and ordered a 5-day suspension of its liquor licence.

The Registrar issued a Notice of Proposed Order to revoke the respondent's registration as a gaming assistant.

The respondent admitted to operating an unlicensed combination poker tournament, raffle, and dinner for profit under a Special Occasion Permit, and to filing false information stating the event was a 'Stag and Doe' not intended for profit.

Inspectors observed patrons consuming alcohol and signs of service well after the permitted hours on two separate occasions.

The Board found the evidence of the inspectors credible and consistent, concluding that the licensee failed to clear signs of service within 45 minutes of the end of the serving period and attempted to conceal evidence during an inspection.

The Board found the licensee in violation of the Act and regulations, and invited written submissions on penalty.

Following a finding that the licensee permitted drunkenness and disorderly conduct in its establishment, the Board considered the appropriate sanction.

The intoxicated patron had assaulted a police officer and another patron.

The parties submitted an Agreed Statement of Facts admitting that a patron removed liquor from the premises and a rear fire door was locked contrary to fire safety regulations.

Based on a Joint Submission on Disposition, the Board ordered an 8-day suspension of the liquor licence.

During the hearing process, the Licensee entered into a repayment agreement with the Ministry of Revenue and subsequently paid the outstanding debt in full.

As a result, the Registrar requested to withdraw the Notices of Proposal, and the Board accepted the withdrawal.

The applicant applied for a liquor licence for a sports bar.

Resident objectors opposed the application, citing concerns about insufficient parking and potential noise from the outdoor smoking area.

The Board found that the objectors did not establish that issuing the licence was contrary to the public interest, noting that the municipality had recently reviewed and approved the parking situation.

However, acknowledging the residents' valid concerns, the Board approved the licence subject to conditions requiring the licensee to advertise off-site parking availability and monitor noise from the smoking area.

The Registrar of the Alcohol and Gaming Commission of Ontario alleged that the licensee violated subsection 45(1) of Ontario Regulation 719/90 by permitting drunkenness on its premises.

During a routine inspection, police and an AGCO inspector found a highly intoxicated patron passed out at a table.

The licensee's manager claimed ignorance of the Ontario requirement to remove intoxicated patrons and gave contradictory testimony regarding the patron's condition.

The Board found the manager's testimony lacked credibility, accepted the licensee's counsel's acknowledgement that an intoxicated person was on the premises, and found the licensee in breach of the regulation.

Following a finding that the licensee breached subsection 45(1) of the Liquor Licence Act, the Board previously ordered an eight-day suspension of its liquor licence and requested the licensee propose suspension dates.

The licensee proposed acceptable dates.

The Board ordered the liquor licence suspended for eight consecutive days commencing April 6, 2009.

The Registrar issued a Notice of Proposal to suspend the liquor licence of Jameson's Restaurant for allegedly permitting drunkenness, contrary to s. 45(1) of O. Reg. 719/90.

A patron, who had consumed alcohol and cold medication, assaulted a police officer and another patron inside the establishment.

The Board found the police officer's evidence credible and concluded the patron exhibited strong signs of intoxication.

As staff took no action to prevent the incidents and allowed the patron to remain on the premises, the Board found the Licensee permitted drunkenness and contravened the regulation.

The Registrar issued a Notice of Proposal to suspend the liquor licence of Zucca Bar Inc. following an incident where police officers attempted to inspect the premises.

The Board found that the Licensee contravened the Liquor Licence Act and its regulations by permitting a patron to remove liquor from the premises, failing to facilitate a police inspection, and permitting drunkenness on the premises.

The allegation of obstructing an inspection was dismissed, as the brief delay caused by a patron holding the door did not amount to obstruction.

The Board invited written submissions on penalty.

The Licensee was found to have violated s. 45(1) of the Liquor Licence Act by obstructing an inspector from accessing the third floor of the premises during an inspection.

The Registrar sought an 8 to 10-day suspension, while the Licensee requested a 3-day suspension, citing a mistaken belief that unlicensed areas were not subject to inspection.

The Board emphasized the importance of lawful inspections and cooperation, and ordered an 8-day suspension of the liquor licence.

The Registrar issued a Notice of Proposal to revoke the Licensee's ferment on premise licence on the basis that the business had ceased to operate and the licence had not been surrendered, contrary to subsection 15(4) of Ontario Regulation 58/00.

The Licensee failed to attend the hearing despite being properly served, and the hearing proceeded ex parte.

The Board accepted the evidence of an AGCO Inspector that the premises were abandoned, dirty, and unoccupied.

The Board found the Licensee violated the regulation and ordered the immediate revocation of the licence.

Following a finding that the licensee contravened subsection 45(1) of Ontario Regulation 719/90, the Board previously ordered a 25-day suspension of the liquor licence and requested the licensee propose suspension dates.

The licensee proposed acceptable dates, and the Board ordered the suspension to commence on April 19, 2009.

The Board previously found that the licensee violated subsection 45(1) of Ontario Regulation 719/90 and ordered a seven-day suspension of its liquor licence.
