Addendum

[1] The judgment dated December 10, 2012 is amended as follows:

1. The words in paragraph 1 “defendant, Roman Cervenak (Cernak)” are omitted and are replaced by the words “Statutory Third Party State Farm Mutual Automobile Insurance Company” so that the paragraph reads as follows:

[1] This is a motion for an order removing the law firm of Reisler Franklin as solicitors for the Statutory Third Party State Farm Mutual Automobile Insurance Company.

2. The words in paragraph 13 “the defendant Roman Cervenak” are omitted and are replaced by the words “Statutory Third Party State Farm Mutual Automobile Insurance Company” so that the paragraph reads as follows:

[13] There will be an order removing Reisler Franklin as solicitors of record for the Statutory Third Party State Farm Mutual Automobile Insurance Company. Hamide may make written submissions on costs within 10 days and State Farm may have 10 days to respond.
