On October 3, 2012, Norman Lloyd DeSouza (“DeSouza”), licence # 122353, was issued Ruling TB ADMIN 58/2012 wherein his licence was immediately suspended.
On April 1, 2014, DeSouza applied for a Reinstatement Hearing
In April 11, 2014, a hearing was convened to hear his appeal and was subsequently adjourned to a later date to allow for receipt of his latest test result.

MR. CHAIRMAN: The decision in this matter obviously it is on consent. Parties have agreed to it. We have the post violation agreement that comes into effect as of today I note. It will be updated to today and counsel has pointed out the stay of the $1,000.00 fine and subject to no problems during the post violation agreement. Anything further?
