REASONS FOR DISPOSITION
(Dated July 23, 2025)
Introduction
Alexandra Moores, age 33, was on August 17, 2015, found not criminally responsible on account of mental disorder on charges of assault and criminal harassment contrary to the Criminal Code of Canada.
On July 7, 2016, Ms. Moores was again found not criminally responsible, on account of mental disorder, on charges of assault and assault causing bodily harm, all contrary to the Criminal Code of Canada.
On June 16, 2025, Ms. Moores appeared before the Ontario Review Board (the Board) at the Brockville Mental Health Centre (the Hospital) for her annual hearing.

In preliminary positions the Hospital supported the patient’s current disposition and advanced that significant threat to the safety of the public was made out and that it was in agreement with the transfer to the Royal Ottawa Hospital. Crown counsel indicated it supported the Hospital regarding significant threat and will be taking no position on the requested transfer.
At the conclusion of the hearing, the Board agreed that a transfer to the Royal Ottawa Hospital was appropriate on the same conditions found in the current disposition.

"NCR #1 (ORB #6812): Harassment/Threatening conduct:
Case File Synopsis documents indicate that the victim, Christine Leveck, and the accused, Alexandra Moores were unknown to each other in 2013. Leveck began receiving a series of unsolicited text messages from Moores approximately two years ago. In some of these messages Moores has stated such things as:
I'm going to come to Kingston and slit your throat while you sleep
I'm going to come and kill your family
I'm watching you
Die bitch
Alien stop messing with my mind
I'm admitted to the psych ward on homicidal thoughts
Assault:
These charges arose out of an incident that occurred in the Emergency Room at Health Sciences North in Sudbury. At the time she was waiting to be seen and she claimed that she was hearing voices telling her to attack people in the waiting room. Eventually she grabbed and tried to choke an 80-year-old lady being brought in on a stretcher. She was restrained from causing serious injury by staff.
NCR #2 (ORB #6984): Assault and Assault Causing Bodily Harm:
On March 23, 2016, Ms. Moores was sitting colouring at a table in the dining room ... Without provocation and for no apparent reason, Ms. Moores reached up and grabbed the staff's hair, pulling a clump of hair from her scalp. Other staff rushed and Ms. Moores now had hold of the staff's uniform top. Ms. Moores dug her nails into the back of another staff's hand. Ms. Moores stood up and began walking towards her room. Instead of returning to her room as she was directed, she approached an exercise bike and attempted to topple it over. Ms. Moores threw herself onto the floor when staff prevented her from doing so. As she stood up, she picked up a linen hamper, swung it and threw it at two staff. The linen hamper was constructed from steel tubing and was on casters. Staff took physical control of Ms. Moores and placed her in seclusion. Ms. Moores was asked to sit on the mattress on the floor and she failed to comply. Once seated Ms. Moores intentionally threw herself onto her back and kicked a male staff in the head. This staff sought medical treatment and was diagnosed with a mild concussion. The female staff had a sore scalp but did not seek medical treatment."

Background History
Ms. Moores is a never married single woman.
Ms. Moores has convictions as a youth for assaultive behaviour and as an adult for assault, threatening, fail to comply with probation and mischief. She choked an elderly patient in hospital in 2014. She has a history of arson. Her behavior includes threatening to kill a roommate with a knife, threatening to kill and strangle her psychiatrist, throwing objects at police, pushing a random woman and wrapping her hands around the woman’s child, attempting to bite staff, and assaulting an elderly woman on public transit.
She was born and raised in the Ottawa area. She is the only child of a tumultuous marriage. Her parents had issues with alcohol and depression. She has suffered abuse, including sexual abuse. She was a Crown ward from the age of ten until she was eighteen. She moved several times. At 19, she landed in Sudbury. She had difficulties keeping stable housing. She graduated Grade 12 and had hopes to become an esthetician.
Her psychiatric journey began at 10 years of age. Ms. Moores does not have a history of substance abuse. She has received several diagnoses.
Evidence at Hearing
Dr. Esther Carefoot, the patient’s psychiatrist, testified. Ms. Moores has suffered from breast cancer in past. It has made a reappearance necessitating rounds of chemotherapy. Her oncologist is in Ottawa.
The report was corrected to update that Ms. Moores reside in B-3 North, the least restrictive ward. In December 2024, she was referred to Watford House, a supported residence in Ottawa. Ms. Moores is cooperative with her treatment regime. She is challenged by a number of physical issues.
The purpose of the Rule 13 transfer is to house the patient closer to her outpatient team.
She has had difficulties with the use of the internet that include shopping and buying at night. This has necessitated Ms. Moores and putting her phone in a lock-up at night.
Dr. Carefoot strongly endorse the transfer and indicated she would be available during any transition period.
Analysis
Significant threat was not in issue and is amply made out on the record, including the hospital report. Ms. Moores is doing relatively well as an outpatient residing at Watford House. Unfortunately, she requires more treatment from a reappearance of breast cancer. Her oncologist and chemotherapy are in Ottawa. The Royal Ottawa Hospital is fully prepared to take over full supervision Transferring Ms. Moores to Ottawa makes absolute sense. Dr. Carefoot will be available in the transfer period. No one suggested any conditions of the Disposition need to be changed.
The Board agrees with the transfer to the Royal Ottawa Hospital. We wish her well as she deals with her health challenges. The disposition with conditions will remain the same.
