DECISION OF THE TRIBUNAL: November 15, 2011

[1]. On October 6, 2011, Sandra Kahu filed an application with the Pay Equity Hearings Tribunal (the "Tribunal") in which she named Walder & McSweeney Contracting Ltd. as the responding party. Ms. Kahu did not file a copy of the Notice of Decision or a completed Statement of Service (Form 3) as required by the Tribunal's Rule of Practice. After Ms. Kahu was advised by the Tribunal of the need to file the Notice of Decision and the Form 3, Ms. Kahu advised the Tribunal, on October 18, 2011, that they would be filed. To date Ms. Kahu has not filed the Notice of Decision or the Form 3.

[2]. Ms. Kahu is hereby directed to file with the Tribunal within 7 days of the date of this decision a copy of the Notice of Decision and a completed Statement of Service (Form 3), failing which this matter will be deemed to be terminated without further notice to the parties.
