Decision of the Board; June 11, 2001

1This is an application under section 61 of the Occupational Health and Safety Act, (the "Act") appealing Orders No. 2 and No.3 in Field Visit Report No. 19644 issued by Inspector Chris Vasey, April 2, 2001.

2The applicant in its letter of April 11, 2001 states: "Detailed report, outlining basis for appeal to follow."

3By letter dated May 2, 2001 the Registrar of the Board advised the applicant that it would not process the application because it did not comply with the Board's Rules of Procedure.

4The Board directed the applicant to resubmit the application. The letter states in part:

"... In order to assist you, we are enclosing a copy of the Board's new Rules of Procedure. Your application must be filed with the Board within 10 working days from the date of this letter. Failure to file the completed form within the allotted time may result in the termination of this application."

5As of the date of this decision the applicant has not responded to the Board's letter of May 2, 2001.

DISPOSITION

6The applicant has failed to comply with the Rules of Procedure. This application is hereby terminated.
