DECISION OF THE BOARD; April 24, 2001
The Board is in receipt of correspondence dated April 17, 2001, in this matter from the responding party. In that correspondence, the responding party states that the applicant failed to include a blank copy of a Response to Application Regarding Employee Status (Form A-42) and the Notice to Responding Party and/or Affected Party of Application Regarding Employee Status (Form C-20) with the application served on the responding party. In the circumstances, the Board directs the applicant to deliver these forms to the responding party forthwith and to file a Certificate of Delivery with the Board confirming delivery. The deadline for filing a response in this matter is accordingly hereby extended to ten (10) days after the date of delivery of these forms to the responding party.
