1. The Board is in receipt of a request for reconsideration of its decision dated September 25, 2001 in this matter. It appears from the Certificate of Delivery filed with this request that the applicant has not delivered the request for reconsideration and completed Notice of Request for Reconsideration (Form C-24) to the other parties, being the responding parties, Galaxy Holidays and the Ministry of Labour.
2. The Board will not consider this request until these documents have been properly delivered to the other parties and until the applicant has filed an amended Certificate of Delivery with the Board confirming that she has done so.
