[1]. This is an employee appeal with respect to an Officer’s decision not to issue an Order to Pay.
[2]. On September 14, 1999, the employee wrote to the Board, seeking a review, but he did not use the Board’s forms or provide sufficient information for the Board to process it.
[3]. The employee was advised by letter dated September 20, 1999 of the problems with the application, and provided with the forms and information to remedy it. To this date, the employee has not re-submitted his application.
