DECISION CORRECTION
On November 24, 1995, I issued a decision in the above matter. On page 8, in the paragraph at the top of the page, it states:
In the case of Polus, a fact situation similar to the present case, Arbitrator Palmer dismissed the applicant's claim for failure "...to produce the medical and financial documentation ordered, or to present any other information to support his claim." Moreover, she found that the applicant's...She noted that the insurer had been forced...and accordingly, she ordered the applicant to pay the insurer its filing fee.
This paragraph is in error. It has been deleted and replaced by the following:

Moreover, he found that the applicant's...He noted that the insurer had been forced...and accordingly, he ordered the applicant to pay the insurer its filing fee.
The second sentence of paragraph 3 on page 8 is in error. It states:
...I agree with Arbitrator Palmer's comments in Polus where she states...
This sentence is deleted in its entirety and replaced by"
...I agree with Arbitrator Seife's comments in Polus where he states..
