DECISION AND REASONS
This matter came on for hearing before a panel of the Discipline Committee on August 29, 2017 at the College of Nurses of Ontario (“the College”) at Toronto.
The Allegations
The allegations against Mandy G. Townend-Kolodziej (the “Member”) as stated in the Notice of Hearing dated August 14, 2017 are as follows.
IT IS ALLEGED THAT:
- You have committed an act of professional misconduct as provided by subsection 51(1)(c) of the Health Professions Procedural Code of the Nursing Act, 1991, S.O. 1991, c. 32, as amended (the “Act”), and defined in subsection 1(1) of Ontario Regulation 799/93, in that, while employed as a Registered Nurse at Princess Margaret Cancer Centre in Toronto, Ontario (the “Hospital”), you contravened a standard of practice of the profession or failed to meet the standards of practice of the profession in that:
a) on one or more occasion, you removed medication from the Hospital’s medication control supply and failed to administer or dispose of those medications as listed in Schedule “A” to these allegations;
b) on one or more occasion, you removed medication from the Hospital’s medication control supply and failed to properly document the administration of those medications to clients as listed in Schedule “A” to these allegations; and/or

You have committed an act of professional misconduct as provided by subsection 51(1)(c) of the Health Professions Procedural Code of the Act, and defined in subsection 1(13) of Ontario Regulation 799/93, in that, while employed as a Registered Nurse at the Hospital in Toronto, Ontario, you failed to keep records as required, in that on one or more occasion, you removed medication from the Hospital’s medication control supply and failed to properly document the administration of those medications to clients as listed in Schedule “A” to these allegations; and/or
You have committed an act of professional misconduct as provided by subsection 51(1)(c) of the Health Professions Procedural Code of the Act, and defined in subsection 1(37) of Ontario Regulation 799/93, in that, while employed as a Registered Nurse at the Facility, you engaged in conduct or performed an act, relevant to the practice of nursing, that, having regard to all the circumstances, would reasonably be regarded by members of the profession as disgraceful, dishonourable or unprofessional, in that between May 2013 and May 2014:
a) on one or more occasion, you removed medication from the Hospital’s medication control supply and failed to administer or dispose of those medications as listed in Schedule “A” to these allegations;
b) on one or more occasion, you removed medication from the Hospital’s medication control supply and failed to properly document the administration of those medications to clients as listed in Schedule “A” to these allegations.
