ORDER
Introduction
1Your claim to the Criminal Injuries Compensation Board (CICB) has been considered. Based on the documentation contained in your file I find you are a victim of crime within the meaning of section 5 of the Compensation for Victims of Crime Act, RSO 1990 c. C24, as amended (CVCA) as the result of a single crime of violence.
2I award compensation to you as follows:
Subsection 7(1)(a) Expenses (Section 14 Award) $1,200.00
Subsection 7(1)(a) Future Pre-Authorized Expense $1,200.00
Subsection 7(1)(d) Pain and Suffering $5,000.00
TOTAL AWARD (AND COSTS) $7,400.00
Less: Section 14 Award – already paid/authorized $1,200.00
Less: Pre-Authorized treatment costs $1,200.00
Total Current Award $5,000.00
3I authorize up to $1,200.00 (exclusive of any applicable taxes) for physiotherapy sessions not covered by another benefit provider (for example your employer or insurance). These sessions must take place within the next three years (36 months from the date of this decision). The Board will pay your qualified treatment provider directly.
4Your claims for dental treatment and hospital expenses are denied because you have not provided receipts or other documentation to support them.
5The amount awarded to you is consistent with awards the Board has made to other victims of crime who experienced similar incidents and injuries. I am satisfied it is reasonable in the circumstances.

7The CICB Orders that the following sum be paid to:
