1This is an Application under s. 36 (1) of the Child and Family Services Act, R.S.O. 1990, c. C.11 as amended (the “Act”) for a review of the Applicant’s placement at [Facility] and a determination of where she will be placed. The Statutory Powers Procedure Act, R.S.O. 1990, c. S.22, applies in these proceedings.
2At the hearing held on June 29, 2015, [the witness], a witness from [the Facility] testified that the Applicant had been examined by a psychiatrist who had dictated a consulting note to be placed on the Applicant’s file by July 3, 2015.
3The Child and Family Services Review Board makes the following order:
[The Witness] is ordered to provide a copy of [the Doctor’s] consulting note, prepared for the Applicant’s [Facility] file, to the Society by July 3, 2015.
The Society shall produce a copy of the consulting note to the Board and to counsel for the Applicant by July 6 2015.
